Once we got the approval to have and stock the Turbo Reaction Boxes in November of 2012, I needed to write a guideline on how to check and exchange the boxes. Instead of re-creating the wheel, I went to IR to look at their guidelines for their reversal boxes. I took their guideline and changed it to fit with our radiology department and our needs. Attahced you will find the meeting agenda and the minutes for the Radiology QA Committee for the month of August 2012, along with the guidelines.
I first took the guideline from IR and changed some of the information to fit our area and needsl Attached is the guideline showing the changes.

I sent my first draft to the stakeholders to get their approval before I took it to the QA Committee. Attahced is this draft.

Then came the QA Radiology Committee for their approval. Attached is the agenda and minutes for that committee showing my participation.

The committee had made a couple of changes, such as making this guideline effective for out free standing centers such as Lone Tree and The Colorado Center.

After this was all approved by the Radiology QA Committee I thought we were done with the guideline, then Kriston Paston (Life Support Coordinator) said that it needed to go through the PPPC. I got in touch with Sandy Godcharles (Quality Improvement Specialist) and she said that it did not need to go the PPPC but probably the Guideline Committee. I did take it to the Guideline Committee and they stated that it should go to the PPPC. Just as I was preparing it to go the the PPPC, I received an e-mail from Andrew Davis (Pharmacy). He was including our Turbo Reaction Box guideline information in the main hospital policy along with the other tackle boxes. So to summarize, it did not go to the PPPC and is part of the hospital policy. I understand that now with Lone Tree and the issues we are having getting our Turbo Reaction Box on the wall in the radiology department that this guideline is going through yet another revision strickly for our stand alone clinics. This is still in the works and I am not sure when it will be resolved. Please see the attached file for the correspondence from Sandy Godcharles.
